In the presence of gross right ventricular hypertrophy (RVH), the depolarising voltage generated in the enlarged right ventricular muscle mass can produce abnormally tall R waves in the right sided chest leads and persistent large S waves in those on the left. Voltage criteria, based on the magnitude of these deflections, have been developed to help diagnose RVH on the ECG. These criteria are discussed in this video.
